Our Transition Home form Hospital is specifically designed to provide support for individuals leaving hospitals to recover at home. We can provide care or support workers depending on your requirements to help you with basic or specialised needs.
Before you are discharged, our Care Assessment Team will assess your needs and propose a suitable Care Plan according to your needs. We will ensure that all relevant services are in place before you leave hospital. We will arrange everything from your transportation from hospital to your food shopping.
We want to help you make you transition from hospital to home as smooth and easy as possible. Help can be short term or long term depending on your individual needs.
The service is also designed to avoid repeat injuries and further injury while you recover. We are here to also help regain confidence that might have been lost due to injury or sickness and help promote independence by supporting you with everyday living tasks.
